Considering both are based upon the same platform, there won't be many differences other than price. The MDX will obviously be more luxurious considering it's more upscale market, and will be more softly tuned. The Honda is more family-oriented, with slightly less quality and refinement, but more value and thoughtful features. Offroad, both are about equal, and both get similar gas mileage. So, hate to say this, but the difference comes down to price.

My girlfriend has an Acura MDX. We went to dinner and she parked next to a Honda Pilot and I realized they are virtually the same car. Considering they are built on the same platform, this would be obvious. From personal experience with Honda and Acura, I have to say that Acura is far more luxurious and usually better on the performance side too. Acuras are usually fully loaded and Hondas usually have different models. The MDX has rich leather, a great Nav system and Bose sound, and relatively quick engine. A top-of-the-line Pilot is probably about 10K less than the MDX. One warning that may only apply to the MDX. It is a gas guzzler! The thing gets awful mileage. I'm pretty sure my V8 Toyota Tundra gets better mileage. If you want a much nicer vehicle and can afford a little extra gas, go for the Acura.

They're almost identical mechanical wise except the Pilot has 244 hp, while the MDX has 253. This has always been the case where the Pilot gets a detuned engine so that the MDX remains a more powerful player. As for price the $44K to $35K difference (top-of-line, with leather and NAV on both) is pretty much just in the options and name of Acura.
The Pilot actually is slightly larger in interior dimension that the MDX, and you feel it. I've only ridden in one but did not get the chance to drive a Pilot, but I have the MDX and for the $9000 difference, if you can afford it, get the MDX. The interior quality and fit and finish is much better in the MDX than Pilot. Also, although it's just buying a name, it's like saying you own an Audi, rather than a Volkswagen.
With above said, have you considered the Lexus RX350? Although costing slightly more, I think the vehicle is overall more superior to the MDX. Also, if you can wait, next year is a brand new MDX and from the autoshow and spy photos, it looks to be pretty much the same look, just slightly more 'aggressive.'

I have an MDX and my friend has a Pilot. I have 38k miles on it, (15k of Manhattan driving). Never a problem, no rattles, hums, vibrations or squeeks. My friend's has 23k on it, no Manhattan driving, 90% highway, he has some of the above. He tells me all the time he wishes he bought the MDX instead. Most features are close (size, power, features, fuel consumption are pretty close to the best in their class, etc...) but quality is the key here. I'm a big supporter of Honda, but Acura really is more refined. And definitely more quiet on the road. You don't have to turn the radio up just to hear it over road noises. Forget about Lexus, more money for nothing, it's smaller and handles like a boat, MDX is more vesatile. Good luck, see you in your MDX soon!!

I have an MDX; my co-worker has a Pilot.
They are virtually the same car. The MDX has a slightly tuned engine that is just slightly more powerful.
Most of the differences are in creature comforts. The interior, in particular, is clearly more refined on the MDX. It is quieter inside when you drive it. It just has a luxury build quality.
Thus, the major difference between the two cars are essentially subjective. I would test drive the MDX first; then test drive the Pilot -- note the difference in luxury and ask yourself whether it is worth the price. Depending on your income level, and how much you drive, the answer could go either way.
(In this sense, the Honda Pilot is an extraordinarily good value; on the other hand, I bought an MDX, so...)
I love both cars. Could not be happier with my MDX.

well, i have a pilot now and i like it alot but later in life id like a acura mdx. ive ALWAYS wanted one..but if u can afford the acura, get it cuz its more luxurious. my father used to say- its a glorified honda pilot. if u like more room, get the pilot. ive heard that the mdx is quite scrunched. also the pilot carriees more ppl. so go with yr gut

they are basically the same..acura in the luxury version of honda..
both just as reliable..and both probly about the same pricing range too.$20-35K..acura will be a bit more expensive..but worth it
build one of each on honda.com/acura.com
