If you belong to SHRM, the Society for Human Resources Professionals, they have many templates that you can use to conduct a harassment investigation. Otherwise, use the intranet and type "harassment investigation" or Human Resources and maybe there's a free site out there that can help you. You may also want to try the EEOC website - they may have something as well. HR.com is also a good site.

Harassment is so very subjective. Someone may be offended by you sucking on your pen or biting your pencil.

not really, your best bet is to go down to your local police department and file a complaint. Thats what their there for!
