They are hit with such force that they are knocked out of their shoes. Sometimes their shoes stay right where they were when they were hit. The thing to understand is that their shoes are not knocked off. They are knocked out of their shoes. Their bodies are moving away from their shoes so fast that their feet come right out.

inertia.. of the shoes... the impact accelerates the victim... but there is not enough force holding on the shoes to accelerate that fast.. so they come off... unless they are tied good.

My shoes flew off! I don't remember; I was unconscious for seven days after but they did. I know this doesn't answer your question but my shoes definitely flew off.
