Maybe the pedestrians not following the signs for walking to a "T". When you are walking across the streets, make sure that you are within the lines, and only walk when you are suppose to!! Do not start walking across the street if you do not have the right of way!!

"Traffic deaths" are one leading cause of deaths, in general.
The leading cause of traffic deaths, anywhere, are accidents.
Maybe your question is "What is the leading cause of accidents?"
Speed and driver error or reckless driving would be my response to that question.
