The gluteus maximus muscle (butt cheek) is the biggest in the human body, that's why. Especially if you have slender upper arms, it will hurt more there.

Shots with a long needle are called "intramuscular" (IM) and our rumps have great muscles to shoot the meds into. The shorter needles (like for diabetics) is call "subcutaneous" (sub-q) and injects just below the skin. So, it depends on the shot, your muscle structure and probably other criteria assessed by medical professionals.

some people freak out when they see a needle, plus since the butt is basically a big blob o' fat (ok, two blobs) there aren't a lot of nerves back there.
and, yeah, doctors are sadistic. why do you think they keep the stethoscope in a freezer!?

Normally it is because the solution needs to be what is known as subqutanious, which is under the skin and fat. With the rear being such a large mass of fat, it is simply easier and a safer target.

Well, they give it to me in the arm but, that is because I'm not a girl that they can look at.

Maybe they just don't want to look you in the eyes when they perform their nasty deed? Maybe watching so much alarm and panic on people's faces takes its toll on them.....so they wait until you are not looking and then jab it in your keister? I should ask them the next time i'm in need of their tender services.

less pain, faster results, and many people fear the needle thus by not being able to see it, they're less likely to have an episode, so to speak.
