Don't apologize and don't explain. If pressed, go through a few rounds of "No, that just doesn't work for me" and "It's simply impossible" and so on. If necessary escalate to "Well, I'll certainly let you know if I change my mind." If they persist beyond this, you might say something like "I'm not arguing with you, I'm TELLING you. My answer is NO and it's not going to change. Please don't mention it again." If even this fails to put an end to the pestering, it's time to say "Good afternoon" in that tone of voice that means "I'd like to slap your fool head off" and put some distance between yourself and the pest.

I have hard time turning people down too. Its because I want to be soooo nice. But if that person won't quit just say NO quietly then if they don't give up then say no loud and firmly.

If someone is being rude, or pushing you into something, cut it off witha quick and clear no.

If someone is raping you or something, shout and scream and kick and bite as much as you can. Who cares about decency then?

If you want to be nice to a person, then state your no in such a way that when they leave you they will thank you.

Make Eye Contact. "No". And perphaps give logical reasoning as to why you're declining whatever offer or gesture was being applied to you.

i think it would totally depend on what the question is, or action requested or demanded, which would lead to how strongly the word NO would be used
