Tell her you will take her out to dinner on her birthday and it will be your gift to her...she will be extremely pleased {take her to a posh restaurant if you can afford it}.

My strategy is to try to figure out either 1) what someone would really like but won't bother to get/do for themselves because of the expense or 2) what someone would like but doesn't know about.
If you live nearby, find a pretense and offer to go shopping with her -- and watch to see what she looks at and puts back with the most hesitation. Then go back to get it.

A hand-written note-a kiss or hug but most a of all an I love you.If you cant do it in person then try to do it by fone but She wont care witch as long as you do it.Wishing her a very happy birthday .None of these can be bought with money and that is why they are special.

Taker her out for the day (or the closest Sat. or Sun. if a weekday is not possible). Take her for lunch and shopping, maybe even a spa treatment if she is into that. I'm sure she'd love the time with her child. Invite your siblings if you have any.

Bring an old photo for herlself, photo where she was young
but pick one so beautiful..
take it to lab, let them do some adobe photoshop with it..
like black and white, or sketching style, or neon glow
make it large, put it in a classic Victorian frame, wood is better.
rap it nicely and placing behind curtains in her bed room, with a note " Mommy, now i don't wonder where i got my beauty from, i love you so much"
Good luck ..

How about a lovely flowering plant? Does she garden? Or a subscription to a magazine about something she likes. Or pay for a massage! I'd love that from my kids.

Give her the picture (photo) of you (or with ur bro and sis if u have). Put it in a nice hand made frame (you could make it from paper and beads). I'm sure she'll love it. A hand made one is always touching and never be forgotten:-)
