I believe it is from the Judeo Christian beliefs that are expressed in the Bible. Men are told not to cover the head when praying. Women are told to. So in entering churches, which in America were initially the town meeting house, school house, etc (multifunctional buildings) the men would remove their hats.
This may be one reason that I can think of, also explaining why women used to keep their head covered (the use of bonnets, etc)

Taking of men hat is an old tradition. Actually Hat is the Silhouette of man. Taking of the men hat is merely acknowledging or showing some respect especially on the street. However, taking off men hat in or inside the building is a descreation of a person who wore the hat.
According to Victoria Seitz PhD, image expert, What happened to manners of taking off your hat when entering a building?Hats are usually worn when it’s cold outside, to a fashion show or for Easter most commonly. Other times include baseball, football, soccer and other athletic events. But please, once inside remove your hat with the exception of a sporting event. I don’t find it so much in business, although interviewee’s feel it is part of who they are and wear them to an interview – WRONG. Hats or head coverings of any nature are not permissible at work unless you work outside as in construction. Hats are for your time and out of respect, remove it when entering someone’s home or restaurant. Wearing a hat inside may be acceptable to some but not everyone. Communicate respect for your friends’ or boss’ home and remove your hat. And when you hear the National Anthem play, show your pride and respect and remove your hat. Whether you notice it or not it will make a lasting positive impression!!!
So, Taking off men Hat is an old tradition as early as the medieval ages as a sign of respect and honor.

Maybe there was a time people only wore hats if the weather was bad but then it became trendy but people still took their hat off under shelter.

I don't know where it started, but I wish someone would tell
my brother that it did! I tell him you should take your hat off
in a restaurant but he ignores me & leaves it on. I am going
to refuse to pay for his dinner again.
