Cheese is really high in fat, so you should try sticking to lower fat types. Light green/water food items such as lettuce, cucumber, celery really have no nutritional value at all and are mostly used for "filler" or "roughage" What I would suggest is adding dark green vegetables such as broccoli, baby spinach (a great substitute for lettuce), green onions, cauliflower etc. They're full of vitC, iron and fiber that the light green/water veggies don't have.

Remember to use low calorie/low fat dressings because you can actually have more fat and calories in a salad than in a cheeseburger depending on the dressings you choose.

I would also add a protein item. The cheese you use is great assuming it's low fat. Adding a boiled egg to your salad 3 times a week is good as well. You can also sprinkle toasted seeds or nuts to your salad for a different texture and flavor cutting your need for too much dressings.

You can try cooked salads. Grilled veggies tossed together to make a warm salad: eggplants, onions, zucchini (or other squash veggies) sweet potatoes, mushrooms all make for great cooked veggies.

Make sure you also add lots of beans/peas and legumes. 1/2 a cup of red kidney beans, or garbanzo beans etc. will up your fiber intake, make the salad much more substantial and satisfying and leave you feeling fuller longer.

When using sharp tasting cheese, you can offset the cheese by adding fruit. 1/2 a cup of grapes or cubed apple pieces taste amazing with sharp cheddar, blue, or asiago cheeses.

If you continue to eat lettuce, carrots and cheese, you will become bored and definitely malnutritioned....mix it up a little...anything you want can go in a salad!

Too few calories. You need more calories, more variety, and more protein. Don't go lower than 1200-1500 calories a day, you body will think it's starving and horde everything you eat. Too little variety, you'll get bored and you're also not getting all the nutrients you need. Add different veggies. More colors. How about some peppers? Cucumbers? Snow peas? (crunchy, sweet, and low-carb. Different from sugar snap peas) I hope you're at least using dark greens, not iceberg! A little baby spinach mixed in the lettuce. Put a little chicken, tuna, or grilled steak on there. And don't forget the dressing! You need a little fat from the dressing to properly absorb the vitamins in the veggies.
