My step daughter is wanting to hyphenate her last name with mine and my wifes so it will be like our other kids but I cant seem to find a form that i can use to petition to the court with. We are in the state of arkansas so any help would be great. I would also like information on the whole process and all the forms I will need. Her father is consenting to this so it wont be unconsented by him. I just need all the forms for the process of this. Please help if you can...

Go to family court and request a name change for a monor. They will make you fill it out then you will probably have to put a notice in the local paper. That gives a person time to protest. Then you go before a judge and if no problems they almost alway grant it. I live in Cali though so some things may be different.

go to the vital records department they should be able to help you if you bring all parties involved they can probably do it on the spot just make sure you bring birth certificates, social security cards marriage licenses and a notarized statement from the bio father /mother saying that you have permission to do this yes you will still need it if the person is with you oh bring drivers license too they will need to make sure you are who you say you are...good luck
