I say it both ways as I've always been unsure about it. If caramel is used twice in a sentence or I have to repeat the sentence I said that had "caramel" in it, I'll use first the care-a-mell form, then the car-mull form. Just an odd quirk of mine, I suppose.
And I guess that you say "care-a-mell".
