A Formal Dinner is served:
1. Soup, fruit cup or melon, or shellfish 2. Fish (unless shellfish is served first) or sweetbreads. 3. Entree, with vegetables. 4. Salad.
5. Dessert. 6. Coffee and after dinner drinks
The number of courses dictates the table setting, as each course will have its own silver.

An informal meal consists of two to three courses including an appetizer, such as soup or salad; main course and side dishes; and dessert.

It depends on how fancy you are getting. If it's just medium fancy, it begins with appetizers, then soup, then salad, entree, and desert, then cheeses (yes! after desert, a cheese tray is passed. I have no idea how anyone finds room for it after all they have already eaten, but that's the drill.)
I once went to a 13-course dinner, where they served sorbets and various other odds and ends in between courses, to 'refresh your palate'. My bottom was numb by the time dinner ended, and my palate had completely given up. But it was the experience of a lifetime.

it depends on how many courses are involved. The most common is 4 courses: the 1st course is salad, 2nd course is an app, 3rd course is the entree and the 4th is dessert

They don't refer to a feast as "everything from soup to nuts" for no reason. Appetizer, soup, salad, main course, sherbet (to cleanse the palate) desert, cheeses, nuts and finally a good brandy.

Slurp, gulp, gnaw, unbutton pants, more slurping, explosive belching, high pressure farting, then asleep on the sofa.

I guess you had to be there.
