I personally have called them both on and off mommy and daddy or dad and mom if im mad i say dad or mom most of the time but otherwise its kinda a mood thing for me.

if it's a case of me wanting them to do something it's mommy or daddy, if it's birthday I'll usually write that on the card too, but otherwise I call them Mom and Dad

I would think it has more to do with how old you are and what your families traditions practice.
Normally, I would think, all children, maybe, what? 10 years old and younger will address their parents as mommy and daddy.
Some children will probably continue addressing their parents as mommy and daddy thru adolescence.
Generally speaking though, I think as teenagers thru adulthood, more often than not, the average child will address parents as mom and dad.
