How do I apply on line for this certificate?

you can't , the E111 does not exist anymore. it was replaced with a card which lasts a lot longer. ( the EHIC) you can still get the forms at the postoffice. and no doubt you can apply online.
a previous reply said it is 12ukp. it is not. it is free as the EHIC and the E111 is based on national insurance.

The E111 has now been replaced by the European Health Insurance card, or EHIC.

i had to get one l;ast week as im heading to canary islands soon. you get the form from the post office and there will be a website where u can apply for it faster. ot will need either your NI number or your NHS number(ur doc will tell u what it is). it onlt took me 4 days to recive mine,so give it a go online,much faster. good luck.x

I definitely recommend using the above link.

I ordered mine online the same day as a friend ordered hers by post. I recd mine within 48hrs, hers didnt arrive until the day after we had gone on holiday.

I think you have to do it through the Post Office.

has this changed to the EHIC card, if so try www.nhs.uk
