The most famous of course is Miss Elizabeth who OD ed in Lex Lugar's apartment in 2003. The 911 call was replayed several times as Lex was panicking. He was arrested for drug possesion. I know Elizabeth was technically a valet not a wrestler but she was probably the best known and most popular in the sport for a couple decades.

No WWE women wrestlers have died in the last 10 years. Those mentioned by the other answerers (Sapphire, Miss Elizabeth and Miss Cleavage) were managers/valets -- not wrestlers.
