I won't know if this is effective for your company. For my previous company, we will send a survey form to the client for them to fill up.

All survey result is to be confidential and submit to the management for assessment. If there is any poor feedback, our management and the staff will meet up with the cilent to assess on the area where the staff can improve.

Personally I feel that this is actually one of the better method if the management is not being biased to any staff

or, the effectiveness of how your company deals with issues after receiving feedback??

or, the effective content of what the customers are providing in their feedback??

they ain't the same thing!
