The same way you do on earth. You still hold on to it and have control of the tampon. I think a better question is how do you have a bowel movement in space.............

Same way you do on Earth. Since you are performing a manual function with your hand, gravity is not a consideration. If it were, you couldn't write with a pencil, flip switches, etc.

They have suits designed for women, but you would still do it verrrrrrry carefully.
