A trans-gender person is a person who is born in the wrong body. An example is a male, with the mentality of a female. Although they are biologically male, they tend to act, speak and dress like females. To an outsider, they look like they want to be a girl, but technically on the inside, they are.
Being trans-gender doesn't mean the person physically switches their sex, but a lot of them do. Some cannot afford the operation. Trans-gender is also not the same as gay due to the mentality of the person.
Hope the anwser helps!

Transgender is an overarching term applied to a variety of individuals, behaviors, and groups involving tendencies that diverge from the gender role (woman or man) commonly, but not always, assigned at birth.
It is the state of one's "gender identity" (self-identification as male, female, both or neither) not matching one's "assigned gender" (identification by others as male or female based on physical/genetic sex).

Trans-gender has 2 meanings.
1) Someone who is or feels they are "trapped" in the wrong body, i.e. they think of themselves as a male, but their physical anatomy is that of a female, and vice versa, someone who thinks of themself as a female, but has the physical anatomy of a male. These people are not homosexual although by their actions they may appear so. i.e. The "internal male" prefers females, but people see a "female body" that prefers the company of other females, and, similarly, the "internal female" prefers males, but people see a "male body" that prefers the company of other males.
2) Someone who has undergone a surgical procedure to "convert" their physical anatomy to that of the opposite sex, thus "aligning" their external gender with their internal gender.

Transgender is used to describe a person who feels that their gender identity (i.e. male or female) doesn't match their sex (which is determined biologically; i.e. what's under their pants).
Some transgender people choose to undergo sugery to change thier sex. These people are then known as transsexuals.
Not every transgender person is transsexual!
