If you feel a thud when on and off the gas, you are probably looking at a bad U-joint on the drive shaft. Get under your truck, and look at the drive shaft connection to the rear end. If it is rusted or if you find fresh/shiney steal, the U-joint is probably going bad.
Replaced with a new U-joint should solve the problem. Other than the U-joint, you would be looking at a problem with the rear end. Now unlike the U-joint, if there are rear end issues, you would more than likely need to take it to a professional unless you know what you are doing.

Could be a bad wheel bearing, if it's that bad it's dangerous.
The tug will tell you which wheel to check.
It could also be a broken brake return spring. (Shoe pull back spring) Brake shoe locking up. Probably rear.
Unlikely U joint, they squeal at crawling speeds. But shouldn't overlook.
Could be a broken motor mount. Test car by holding brake hard pedal. Put car in drive and without allowing it to move, give it gas and feel for thump. Also try it in reverse, gas and slack off, Thump that way? Thump is caused by engine lifting up and dropping back.

Might be the overdrive kicking in and out around 45 MPH mine tries to go into overdrive and then jumps back out. Ben up there is right though, I have a 99 explorer and it has tons of problems. If I could give it away I almost would.

The car is miss-firing. The fuel/air mixture is probably wrong...the mixture is too rich or lean when you have to accelerate and "load" the engine to get up the hill. Replace the mass flow sensor and air filter and clean the throttle body. If you haven't replaced the plugs and wires...do that too.
