Maybe a little of both, take some time and lay there let him experiment with you, lay there and have him using his fingers feel around both on the inside and outside of you when you find something that feels good keep on it for a while then move on as he goes he'll find spots that he has to hit if he wants you to come.
Thursting is not enough. Is is an example of it's not the size but how it is used.

It might depend on your age. Younger women often have a harder time having an orgasm. If you can have one by yourself, but not with your guy then it's probably him. Try doing more foreplay like having him build anticipation to get you so turned on you can't wait for him to finally touch your privates. Also, have him read a book and figure out where the G-spot is. When it comes to orgasms oral sex is the way to go.

To make a girl orgasm, finger the G spot furiously (or use a G spot toy), masturbate the clit fast at the same time (like rub a powerful vibrator body massager on your clit at the same time). Maybe have an object in her a-nus (a turn on by women). You have to be horn*y in order for the orgasm to work, so you may want to incorporate goat weed or any good aphrodisiac (a sexual libido enhancer, NOT Viagra or "size pills") . The G-spot is on the top inside of the vagina. Curve your fingers up when fing*ring. For a libido enhancer, try http://www.directvideoanddvd.com/listcategory_toys.asp. Maybe pop in a porno. Girls love when you rub the inside of her thigh(s) while you do this. [for the guys to do]

Well i don't know. Have you ever had a orgasm with another guy? If so then i would say that the problem is with your guy. Then again maybe you just need to relax and and enjoy him.

Do you know what an orgasm is? Cause its kind of weird not to have one at least most of the time. When you do get one what is different about what you or he is doing?

hard to say ( sorry, no pun intended ) could be you. could be something psychological. can you have one on your own ? I was with a girl that I dated that seldom did when we were together either. ( she could get herself off ) we even had a few 3somes and even 2 guys couldn't get her off, so it wasn't the guy's fault. are you taking any meds ? sometimes they will affect you in that way.
If it is a serious problem for you or your b/f maybe you should looking into seeing someone about it. If it ISN'T a serious problem for either of you, but you just want to (and if you CAN do it yourself) then by all means go that route ( I'm sure you b/f won't mind watching as long as you include him ) [ if you do it w/o him, I'm sure that eventually he's going to become resentful...]

would u like to elaborate on this?
well maybe U take the lead and be on top. U can pace urself and take control of the situation. do as u pls till u are satisfied(orgasm). enjoy the moment ,do not think " i want my orgasm!"
u'll only distract urself.... in fact dont think at all, just feel it!
have fun!

It could be you or the guy or a combination of the two. It could also be medically related therefore I would suggest talking with someone close like a parent or a friend or maybe consult your local family doctor.

There actually many women that are unable to climax during intercourse. Other woman just take longer due to nature, or stress, or having problems concentrating. If you are able to climax on your own, then get with your guy and try to figure out what can work to help you climax with him. Trust me, he will be more than willing to help you out in that sense.

Try relaxing. IF you are tense you won't. Just lay down for a few minutes before and take your mind off of everything but. Oh, and don't expect it to happen.
