The two main problems with scenario you describe is where to run the power to the fan(since there is no crawl space to hide it in) and hanging the fan low enough so the blades don't hit the ceiling. That problem can be solved by buying a fan with an extension for that type of installation. To get power to it you will have to run conduit along the ceiling to the fan location...not pretty but not much choice.

The only ceiling fan that you cannot mount to a vaulted ceiling is called a low profile or snug to the ceiling, any fan with a down rod will mount to a vaulted ceiling. What ever type you use the blades should be about 7'-6" above the floor to be the most effective.

You must use a support made specifically for ceiling fans. Your local hardware store or building center can direct you to this.
If there is no light fixture there now, you will need to run electricity to it. Contact a licensed professional for this.
Once the mounting hardware is in place, you can hang your fan from a "downrod", often included with ceiling fans when purchased new. They can be bought seperately as well. The steeper the ceiling, the longer the downrod should be to prevent the blades from hitting the ceiling.

Some ceiling fans come with built-in adapters that can be used on a slant or adapters are available for some models of ceiling fan to be used on a slant. It really depends on the type of ceiling fan. One example of a fan with a built-in adapter can be found at:
