call your previous jobs they have to keep copies. they will send you a copy of the one they have. I have don't it a few times because i cna't keep up with anything

write them a nice letter requesting the info. if they ignore that send a registered letter telling them your going to get a lawyer. then call legalaid and see what they tell you
