First, apologize profusely to the insulted person. After you've taught your child it's wrong, you will have to be more firm and tell her to apologize.
The next time she does it, you will have to speak immediately and firmly to her and tell her that it is not nice to say things like that, and she must apologize NOW. Repeat the process every single time till she gets it.

When my boys did that I explained to them that people come in different sizes, shapes, and colors. I thought it was important though that they understand that what they did can hurt others feeling though. I asked them how they would feel if someone said they had a big head or a big nose or something like that. They said they wouldn't like it. So I explained that other people don't like it either when they point out their differences either. Now they don't do it very much at all. Only to say something like how pretty someone is or say something nice about someone.

Tell her that that is unkind and that people come in all shapes and sizes. People can be heavier or smaller, taller or shorter. Just explain to her that all people are different and everyone deserves respect even when they are different.

You CAN say to her that if she wants to talk about how someone looks because it's funny or different that she should whisper it to you. My son did that way too much, and telling him that worked. Oh, and God didn't make anyone fat, people make themselves fat.
