Just write a letter to your boss (supervisor) and explain that you will be leaving your job in two weeks (and give the exact date which will be your last day of work). Thank him/her for the job and hope you can count of them for a reference if needed in the future.

if you mean to quit a job then just write a letter saying that as of today (whatever date you will be presenting the letter) i am going to be leaving this job and this is my 2 week notice.Fill in the empty spaces but you get the idea.
Make sure you give it directly to your boss and be there when they read it so they cannot say they did not recieve it.

just adress it to your boss, explain to them that its time for you to move on and that you want to show your respect for his/her leadership by giving them the respect of 2 weeks notice

All you need to say is that I am resigning my position effective "the date". No explanation is required.
