Not personnaly - but I've seen plenty.
Australian Magpie's - during breading season anyone who walks past their nest will be attacked usually. They swoop and peck at the back of your head. We have a whole section of pathways at my university that has warning signs all over it during spring. There is always a pile of sticks at either end of the paths - if you hold a stick above your head they won't swoop (or at least not as often).
There is a theory that if the bird thinks you're looking at them they won't attack. So you quite often see people riding bikes, or walking along wearing helmets or ice cream containers on their head that have eyes painted or stuck on the back. This is to trick the bird into thinking you are looking at it. a lot of people ride bikes with those orange flags sticking up from a pole on the back too.
Note: I'm not joking - this is all true!!
My own parrot, a corella, bites everyone who is not me. Which is funny sometimes but not for the person being bitten!

Yes. I think it was actually trying to get my cat though. I have a cat that isn't afraid to climb trees and raid the occasional bird nest. Then again, maybe the bird just wasn't digging my do.

I have a small farm and I can say that I have been attacked by birds quite a bit. We had this one bourbon red tom turkey and he was EXTREMELY mean! He would wait till your back was turned and flog the hell out of you. He got out of the pen and ran off one day, so needless to say he ended up coyote food.

Yes. I was installing an in ground sprinkler system. I don't know what type of bird it was, but it kept swooping at my head. I think it was protecting a nest.

Sure! If you want to try it out, get someone with chickens to let you gather the eggs... For a quality attack experience, Rhode Island Reds are the breed of choice for me!

Well, not in the head, but once I walked by a crow's nest, and their nest had just fallen to the ground with all the chicks in it. The two crows were protecting their youngs and they attacked me and my dog because we were too close to the fallen nest.

OMG!!! Yes!!! This is so funny now, than it was then. I was pressing my lips against the cage making kissy sounds, and they both flew 2 my lips and started ripping away and screaching like bats from hell. It was awful. My mouth was stuck and I was screaming and crying. My mouth was bleeding and I couldn't eat and drink for 2 days! That sad part was that they were only #$#$% parakeets.

I have an Afircan Grey who hates women and will kiss me one second they try to rip my face off the next. I have a Greenwing macaw who didn't like me when she first met me and took a chunk out of my arm. Thank god for my sweet Umbrella Cockatoos....they just wanna cuddle all day...
