I think you should over look it. He pro bally did not even realize what he said. When you are with someone for a long time, it is only natural to say their name by accident sometimes.

glare at him. Ignore him. beat him. make him beg for your forgiveness.
make him never forget your name again.
If he still continues to do it ... start calling him by your ex's name or just get rid of him
