When I am trout fishing on the river, I use a little white jig with a piece of pink yarn tied to its head. In a small lake for bass, I go live bait all the way. When I hit Lake Michigan, it's crawfish tails sliced open lengthwise and hooked - have cought some monster salmon that way. Salt water I stick with different lures, again, depending on the type of water body. I can't speak to using live bait in saltwater with the exception of using smaller fish to catch larger fish, which I also employ when ice-fishing.
In the end, it all depends on what you are fishing for and what type of water you are in.

It really depends on the species, the time of year, water conditions etc. Knowing what bait to select comes with years of pleasant experiences.
That being said, the best bait to catch Queer fish with is foreskins. Go to Mount Sinai Hospital on Tuesday and you can get a sack full of them.

It depends on where you are. Usually whatever grows locally, because the fish are used to eating it.

Having said that I've always found fish go nuts over prawns (you might call them shrimps) that have been left in the sun for a day. Just a little on the end of the hook.
