Does the dog have other health care issues as well? Older dogs can get confused just like humans get Alzheimer's. Discuss this with your vet as soon as possible. Also, make sure the dog is not in pain. My poor old terrier shows signs like this at the age of 17. She also seemed to be in pain - and acted like her head hurt, so my vet prescribed a special kind of aspirin and it helped.

Constantly walking in circles can be a symptom of hydrocephalus which an excessive buildup of fluid on the brain. Sometimes the disease can cause the dog to act disoriented as well.

I want to congratulate you for raising your dog to a ripe old age. That shows great care.
Dimentia is a possibility. Dogs can definitely aquire that in their old age. Other possibilities could be a stroke, or brain damage from seizures. Maybe even possibly a tumor. Does his head tilt to one side constantly? I am sure it's probably just old age doing this, but if you notice tilting of the head to one side constantly, I would bring him to the vet

Are you sure she hasn't had a seizure? Seizures and strokes can cause your dog to behave strangely, from walking into walls, to circling, to barking and growling when nothing is in the room to screaming. If your dog is showing any of these signs take her to the vet to get checked.

I hate to say it, but this sounds like maybe a stroke. I would call a vet, and ask to be sure, but seizures and stroke can very easily cause what you are talking about. Good luck, and remember at 18, you have obviously done right by him regardless. IMHO.

I'd say blind or lost hearing, some senses are going or gone. try to see if it will follow your finger or hear a snap or your fingers.
Most likely dementia has set in and if so you should say your final farewells and have it put to sleep. you owe it a dignified departing for 18 wonderful years.

older dogs can have dementia. there is medication that you can get through your vet that can help . I would recommend taking your dog in for a check up to rule out something else.
