If you refuse to register, it will hurt you for future employment (you can not register after the age of 26) due to many employers requiring proof of registration. Immigrants that do not register while processing for citizenship will not be granted citizenship. You can be refused federal grants, loans (including student loans) and can also interfere with purchasing a home. It is a misdemeanor offense in most states, a felony in some.
I don't know why anyone would worry about it. There is no draft, and probably won't be, unless the protesters stop people from enlisting.

There is no, and will be no, draft!!!! If you fail to register for selective service by 28 then you are inelligble for a lot of government loans, grants, etc. If a draft were instituted & you failed to respond when selected for service then you can be sent to jail for a term longer than your military service would be & with just as much chance of getting shanked in federal prison. Or move to Canada & wait till some panzy president grants you a pardon, then come on back so I can personally kick you in the nuts. Anyway, why wait around, there's no waiting at your local recruiter. Just join the navy if you don't like getting shot at.

If you refuse to register than you cannot get things like student aid, or a government job.
If you refuse the duties of citizenship - why should you get the benefits?

From the Uniform Code of Military Justice:
885. ART. 85. DESERTION
(a) Any member of the armed forces who--
(1) without authority goes or remains absent from his unit, organization, or place of duty with intent to remain away therefrom permanently;
(2) quits his unit, organization, or place of duty with intent to avoid hazardous duty or to shirk important service; or
(3) without being regularly separated from one of the armed forces enlists or accepts an appointment in the same or another on of the armed forces without fully disclosing the fact that he has not been regularly separated, or enters any foreign armed service except when authorized by the United States; is guilty of desertion.
(b) Any commissioned officer of the armed forces who, after tender of his resignation and before notice of its acceptance, quits his post or proper duties without leave and with intent to remain away therefrom permanently is guilty of desertion.
(c) Any person found guilty of desertion or attempt to desert shall be punished, if the offense is committed in time of war, by death or such other punishment as a court-martial may direct, but if the desertion or attempt to desert occurs at any other time, by such punishment, other than death, as a court-martial may direct.
