Better check with your state labor board on this one. Here in Ca, we have SDI which covers that.
If the mileage, etc was during her leave - no - you do not have to pay that. If it was before the leave - do check with the state labor board.

In my company we call that the "new mommy syndrome". They go out on maternity leave - use all the disability benefits - then give notice when the benefits run out.
No, you cannot charge back the insurance premiums as the expectation was that she would be back.
As far as mileage goes or whatever "other misc" goes - if it is owed to her - then you have to pay it.
However, this mileage and "other misc" needs to have been a viable expense before the maternity leave. She cannot charge you mileage and "other misc" while she was on the leave.

I don't think so, but I also don't think you should pay her mileage or whatever misc stuff she asked for. Try to be civil about it and explain to her that she cost the company a lot of money by not coming back when she said she would.

If the leave fell within their provided benefits as an employee, then no. If you extended benefits to her beyond the normal limit, then determine what the cutoff would have been if she had come back to work, and deduct it from any remaining pay.

Does your company have an attorney or in-house counsel? If so, I'd ask this question to that person. All states have different laws regarding this sort of situation, so I don't think you're going to find the correct answer here without giving more specific information.

you can't make her pay insurance if it was a company paid program. also, if her mileage was accumulated while she was on the clock and it's company policy to reimburse for mileage, then guess what...you're paying for it.
i would take this moment to visit the FMLA website and your company's policy on maternity leave.
FMLA has stipulations like a 12-week leave...if the company offers more, then great for the employees.
basically, it sounds like you got a bit screwed....sorry.
