Excellent question. What you are refering to is "androgenic alopecia" or "male-pattern baldness". Of course, there are other types of scalp hair loss which does not follow this "pattern". In fact there are disorders where hair loss occurs on body areas other than the scalp. Androgenic alopecia, is thought to occur in post-pubescent males who are gentically predisposed for the the condition. A population of hair follicles (typically on the top and in the temporal regions of the head) undergo miniaturization and atrophy under the influence of a hormone called dihydrotestosterone (DHT; a metabolite of the male hormone Testosterone). DHT levels in the blood start to rise at puberty and stay elevated (at their normal adult levels) well into advanced age. We don't know what causes this population of follicles to respond in this manner to DHT but research continues. A breakthough will likely win a Nobel prize (I speculate).

Baldness can start anywhere. There is no set pattern for how a head will lose hair.

It is possible to start on the sides but most of us don't see it too often.
