Yes. This isn't like a gift where the thought counts more, it's a monetary token of friendship or appreciation. People don't tend to harp on amount of cards, but do moan when they have to find out before they use it or find out it's too much/not enough for purchase.

Yes, Most gift cards that I have purchased have a spot on the back where you write the amount the card is worth.

Yes. Because it is very irritating for the one receiving the gift card to know how much they can spend before they get to the check out line.

There is usually a space to write the amount on the card. I would write it on the card. If there isn't I would not write the amount in the note card...that would look tacky.
