The response is individual and unpredicatble. Watch the film "Kinsey" for one version. Cinema loves to make out losing virginity is painful because it often is the cumulation of the female character submitting to the male character as a plot device.
As for me - 0 - it did not hurt in the slightest and felt just right.
Fear of pain and not being genuinely stimulated will leave a woman/girl physically unready for penetration and will not help.

It's different for each person and it depends if the woman's hymen has already been broken through exercise, tampon use, or some other means. I would say for most people, it would be 4 or under though. It's not something to worry about.

Depends on the person. It really didn't hurt for me the first time around. But then again, I lost my hymen through horseback riding so much when I was younger. I'd say it was a zero for me.
