i once looked at a caution sign on a matress and it said do not put contents in mouth. how is that possible!?

You're thinking of the big picture like most people do. That label was put on there meaning do not put any part of this mattress in your mouth. Meaning, if it happens to be destroyed don't put the foam, springs, wood, cloth, or anything else used to compose it in your mouth.
