Agree to make a change for the better. If one is not willing to change then forget it. Sit down and make a list of things of reason you feel the relationship is an issue and how it is a success. Why are you together? And look at the reasons she has had to give you chances. Are you dragging your feet?

You can't expect someone to stay with you after they have given you so many chances to change. She deserves better and I think she knows it. This will be a hard lesson for you in the end, and that is to lose the love of your life.

I mean she have gave you so many chances!!!...
Take yourself a decision... let it down...
Or go for it for once and ever... And just in case... Be sure you can make it up... because when you have come back and forward many times the relations is already off, even you think it is not...

Try to have better communication. Never sleep until your arguement is solved. Or try getting counseling. Try spending time with each other in a new place where nobody knows your business and try talking things out. Listen to your partner, then tell your partner what you have to say. Don't interrupt on what is the one trying to say.

Whats wrong with the relationship? Whats the problem? What do you want? What does she want?
