Grandmas love to have their nails done and a pedicure is just the BEST! Maybe get her a gift certificate for one or the other or both! A pretty houseplant would be nice with maybe a pretty card just written with your words to tell her how you feel. A good book with a pretty bookmark inside of it, or flowers, perhaps a Spring bouquet. You are Sweet to be doing this for her!

How thoughtful. Not sure if she's living alone or not, but spending quality time with her is great. If you drive you could offer to do errands for her. If she's not able to get around very well, you could offer to clean her house, mow the lawn....stuff like that. Not sure if that's feasible for the situation or not.
So here are a few other ideas.
Get an assortment of cards (birthday, sympathy, get well, etc.). Go ahead put her return address on the evelopes for her or make her some address labels. Also include stamps.
Fresh flowers are also very good. Grocery stores have set of 3/$10 a lot of the time.

First off, 50 years old, does not make for an old grandma. So just about anything would do. A gift certificate for the mall would be nice, or perhaps something that she could do with her grandchildren, all grandma's enjoy spending time with them.

do something for her, like draw her a bubble bath, make her a sandwich, take her out on a date to see a movie with her favorite movie star (Sean Connery, Pierce Brosnan and Harrison Ford) these are some she might like. and of course a big loud smooch and a heartfelt big thank you these are only a few of the things she might enjoy

Grandma's like it when you spend time with them. Take her out to lunch or a stroll in the park. She'll love it.

Make a scrapbook page or hand made birthday card with a picture of you and her in it. Take her to lunch at her favorite "old person" restaurant or cafe.
