condoms work about 98% of the time. some of the times that they don't work are: if you put it on too tightly, then when he comes it bursts at the end; if the condom is too big, it can come off in the girl; if the condom is old or has been left outside (in the car, in a wallet, etc) then the rubber can get brittle and break...

Anyway, there are several reasons why condoms dont work. But when used correctly, they are 95% effective. But they can rip or tear if not properly lubricated. Or hell, they can just slip off if not rolled all the way down. And NEVER use petroleum jelly for lube, like vaseline. It breaks down the rubber and will increase the chance of breakage.

One way to help avoid having them break is to be sure to buy lubricated condoms. They have a lubricant on the outside to make them "slip and slide" easier, with less friction, so less likelihood of tearing.

No. HIV can pass through animal skin condoms. The best type of condom, for protection from STD's, is one made of latex.
