If you're looking to catch her off-guard, you have to do something "normal" so she won't suspect anything. For example, if you never take her to a fancy restaurant, you shouldn't take her to one to propose because she'll know something's up.
If you're ready to marry her, then I assume you know what she likes to do. Do something that does not completely stray from your normal activities with her so she'll think you're just going to hang out as usual.

Somewhere romantic - a picnic maybe, hot-air balloon etc. Or go back to where you went for your first date or fave restaurant. Remember the ring, flowers, balloons and chocolates - these are easier to give if in a restaurant!
Get on one knee and ask "Would you marry me?"
