How can a bird hand, for example, with digits two, three and four evolve from a dinosaur hand that has only digits one, two and three? That would be almost impossible.

The current idea about this (based on the fossil evidence) is that feathers first developed on smaller and more agile dinosaurs who would jump into the air to catch flying prey (like insects). Initially, these feathers would have been a way to help conserve body heat and keep these dinosaurs more active in cooler times of the day, as reptiles don't maintain their own body heat and extra insulation would have helped reduce their rate of heat loss.
Over time, the arms and the feathers on them would have developed more to allow the dinosaur to stay in the air for longer than just a few seconds while jumping up to catch their food, letting the dinosaurs glide for short distances in the air. And even further on, this would have developed even further to turn the arms into wings that were capable of lifting the animal off the ground and keeping it in the air.
Some people think that it was the pterosaurs (the flying reptiles) that became birds, but this is not true. The pterosaurs were a much older group that evolved flight in a totally different way, by using skin stretched between the arms and legs (much like the bats of today).

Dinosaurs were land animals and didn't have wings, though the t-rex is thought to be in the bird family. Now, flying reptiles, such as the pterodactyl, had wings. Animals are constantly evolving for the better to adapt to its environment. Does this help?

They didn't, birds came from just a tiny part of class dinosauria, not all of them.
To know how this happened, search for information on Archaeopterix sp.

After the great flood they evolved over a thousand years. Only god could truly explain exactly how we evolve.

There are actually fossils showing the evolution of raptors to pre-birds, so we don't have to guess at this one.

Well, they had feathers first. Presumably there was a leaping stage, like the "flying" frogs, snakes, squirrels, and lizards have now.

i guess the environment favored animals that had light bodies and arms that moved quickly to move about faster than other animals.
nature and natural selection favor animals with abilities that allow them to survive.

If your looking for the scientific equivalent of a christian answer; it doesn't exist. We never say something as stupid as: "god did it".
However, if you are honestly curious and would like to read about it, I've put a link for you about transitional fossils. Please read it and try to understand it.
