Honey, just tell him straight up. Chances are that him not doing what he says he will, hurts your feelings, so just tell him. Be honest but not rude. Sit him down and let him know that him being a "flake" bothers you. Also tell him that it gives him a bad image, it makes people not want to rely on him because they know that he will not come through for them. So tell him that it not only hurts you but that it also hurts him as well.

For God's sake, tell him that it bothers you when he doesn't follow through.

The only way your going to be able to tell him, is come straight out. Tell him how you feel when he does not do what he says he will do. Offer to make him a list, so that he doesn't forget. If that doesn't work, and he continues to disrespect you, it's time to KICK HIM TO THE CURB!
