This is a complex answer so I will probably miss some points:
A bad mom is one who doesn't try her best to be a good one, but even good ones are not perfect so you need to try to not be too hard on yourself.
So let's talk about what would make a great mom, since the opposite would possibly be a bad mom:
A good mom -
- Is loving and caring and places her kids' needs first, without making them the center of her entire attention. (told you the answers are complex). What I mean by this, is that kids who are overindulged - mom jumps to answer every need - are not getting their needs met, because it isn't healthy for a child to be raised to be selfish and self-centered, thus sometimes mom needs to say "no," and it is the best thing for her to do in that situation
- a good mom cares about nutrition and not only feeds them with healthy foods (yes..a good mom feeds vegetables even if she's not fond of them), but teaches them as they grow about good nutrition
- a good mom reads to her little ones every night
- a good mom sets limits and boundaries and corrects children who violate those limits and house rules.
- a good mom understands the difference between teaching correct behavior and punishment. Children are not born knowing right from wrong and it is our job to teach kids the difference, and when they make mistakes, to offer up, first of all, the right way to do something, and, if necessary, additional information (that may or may not include punishment) so they remember.
- a good mom respects her children as individuals and expects them to be equally respectful of others
- a good mom gives kids guidance, then lets them make mistakes so they can learn from them
- a good mom teaches by example - be a good example to your children by following the golden rule and not doing things you don't want them to do
- a good mom keeps her promises, even when she made it by mistake
- a good mom has limits, expectations and punishments that are consistent and her kids know it
- a good mom is involved in her child's education, supervises (not does) their homework, asks questions of teachers and supports her children's teachers as necessary
- a good mom gives reasons for the moral lessons she wants them to learn
- a good mom is open to listening to her kids
- a good mom knows she also needs to take care of herself and her marriage, both to set a good example for her children, and to keep herself happy so her children have a happier mom

If the mother doesn't care, she's the worst kind. But if she's a mother who is just trying to do her best because she DOES care, then cut her some slack. Only a mother who doesn't care what her child feels, or hears, or sees is a bad mother.

A lot of things. A bad mother can be verbally abusive, physically abusive, or neglectful. That is a hard question to answer. Can you be more specific?

Well, hitting, namecalling, never having any involvement with the kid, drinking, pills, always dumping kid off with abusive relatives or foster homes, etc. That's bad.

if a mother sets no boundaries, they never say no, they buy their child/teen drugs or alcohol, never punish their child.

neglet, shows no interest in what their child is doing, is verbally or physically abusive, has no clue where her children are or what they are doing.....things like that.
