what about best rewards account and where do i get a w-9 form at?

I am wondering how and when I can get my rewards from your best rewards account. It didn't show up when I was looking through the items listed on the website. I am also wondering where I can get a W-9 form to fill out so that I have all of the information correct, when I send it in. So you let me know if I need any other information please let me know/
