A reading log can have various different formats, but all usually include: Title, Author, Genre, Date Started/Finished, Rating, and (sometimes) a Brief Description of Book.
Genre is basically the "category" in which the book is in... there are memoirs, (auto)biographies, non-fiction, fiction, fantasy, science-fiction, realistic fiction, mystery, etc.
