For 5'8" and 200 lbs. you DON'T need bypass. I was 220 and the same height, and lost 60 lbs with this diet in 3 months and another 15 with only exercise.
For breakfast: 1 bowl oats with 1% milk and splenda, no sugar
mid morning: 1 bowl fruit, except watermelon
lunch: boiled or broiled vegetables, and 3oz. beef, chicken breast or fish. You can have 1 TSP olive oil any condiments you want.
dinner: Salad with 1 protein (1 hard boiled egg or tuna, for example)
I'm not giving you quantities, because my physician said that it's better to eat the amount you need so you don't cheat. He's one of the best doctors of Internal Medicine and is Board Certified, so you don't have to worry about anything. Can have coffee, tea or sodas, just with splenda.
Might sound boring, but as soon as you start, you'll lose fast (because of the extra weight you have). When weight loss starts to slow down, head to the gym for cardio or power walk-eg: walk fast.
If you want, send an e-mail, be glad to help you. I went from size 14 to 8.

Cut your portions down and increase your activity level. If you have a decent appetite, try breaking your intake into smaller meals and pay attention to the time of day that you seem most energetic and get some activity then. Start small, Always! and increase the activity level over time. Be careful to understand this, as this is the biggest factor as to why we put pounds back on when we've reached our desired goal. When you decrease your food intake, your metabolism will slow down since your system is not getting the same amount it used to. Therefore you need to keep up the activity to get your metabolism back up to keep the pounds off. An easy way to get things going, go for a walk, and gradually work up to a pace where you can't sing comfortably. Get to this pace, hold it for five minutes to begin with then increase this time to ten minutes and so on.
