On the cross Jesus says to one of the men beeing crucified next to him "Today you will be with me in paradise."
The question you have to ask yourself is this:
Would heaven be a paradise without your loved one?

I would recommend that that you read the King James version of the Bible. It contains the answers to your questions.
God the Father knew you before you we born in eternity past and when you were formed in your mother womb. We are born with a Soul that will live for eternity. We must decide as a free will agent to accept Jesus Christ as our Lord and Savior or not.
Our decision determines whether we spend eternity in Heaven or Hell.

Actually, we will exist on earth as our main home. This is a common misconception. Humans were made to dwell on earth. There will be a new earth. Pick up a book called, "Heaven is a place on Earth" by Michael Wittmer.

Evidence, yes if you believe in the BIBLE "THATS THE BOOK FOR ME". But you should not conclude it for yourself. I trust the professionals. I think they go to school for what two years to become preachers?
