Your mother-in-law is as good as your own mother. She is a religious mother after you married her daughter. If you can understand the bond between you wife and your mother-in-law, you need not be explained anything further. It is always rewarding to help a sick person irrespective of your own relative or somebody else. A sick person always needs help. You must support your mother-in-law during her sickness considering she is your own mother. You will be rewarded by the God for this act of kindness.

Your uncertainty is a fact. Seems you feel obligated. Helping your Mother-in-law is a very good thing, even if you have reservations, it's the right thing to do. Family can be wonderful, impossible,and you can't get away from it. BUT, unless you have issues with her that is putting a stop sign in front you, again, it's the right thing to do. It depends on how was she was behaving with you before getting sick. If she was loving, caring... why not ?
You are the only one who can answer this question.
But it seems that you do have doubt about helping her.
So, I guess, you do feel obligated just because she is your mother in law and you are worrying that it could be guilty to do not help her.
In fact, it is up to you.
