There are special nipple cups she can wear during her pregnancy to help expose her nipples & if they don't come out enough, she can wear nipple shields during the first few weeks after the birth. After a few weeks breastfeeding she should be ok. Using a breast pump during pregnancy will help bring out her nipples but it will also use up all her colostrum. Colostrum is the important first milk which provides more nutrition and immunity against disease than the milk that comes after it.

Ask her OB/GYN for advice on this. They do make some kind of cup like device the women can use to draw the nipples out from the inverted position. I had a friend that used them while she was pregnant. Also, the action of the baby sucking may pull them out naturally.

As time goes on her nipples should come out a little more. There are also little plastic cups that go over the nipple to encourage them to come out. Have your wife talk to a lactaion consultant, or someone from the La Leche League. They should have all the information she would need.

There should be a lactation person at the hospital to consult and they may offer something called a nipple guard. It worked for me.

There should be a lactating nurse or consultant at the hospital that she plans to deliver in. They should be able to provide her with individualized instruction and support.
