There are many techniques, but the three most popular are :

1. Through the lips -- run the hook up through both lips
2. Hook between the mid-section -- run the hook through the midsection between the eyes and fin.
3. Through the tail-- run it through the tail(not the fin) I Personally don't like this technique because the minnow can not swim

Through the lips is the method I prefer, because the fish has the ability to still move around like a fish and it does not die as fast. But you have to be careful not to sling the bait off the minnow will come off if cast to hard.

Depends on the type of minnow. Shad are best hooked through the lips because they are so easy to kill, shiners you can hook right through the middle or towards the tail.
