Someone in a forum wrote that it first came about in the US in a novel in 1921, and was also related to fighter jets in WWI. I don't know how valid this is, but feel free to check it out at the link below.

My guess would be since hell used to be thought of as underground (there are people who still think it is under the earth)
and bats presumably look like demons, that when bats come flying out of a cave (extremely fast), it resembles demons flying out of hell. Thus "like a bat out of hell."
