When you got your card it came on a little sleave of paper with your pin and card. on that paper it you smart serve number was also printed, so if you kept if for future reference, you can get the number and then go to the smartserve site and get a replacement it's 8 bucks I think. Smart Serve is only good for 5 years now, so if it's older then 5 years you'll need to get another one anyhow, you can see how old it is by the first 2 numbers represents the year if you still have the paper, if not, you'll have to take it again ;(

Yes, Have T-Mobile send you a new one (or go in to a T-Mobile store). You'll probably get charged for the new one. Also, if someone "found" your SIM card, they could put it in any GSM phone and make calls and/or access the internet causing your bill to be outrageous. If you haven't already done so, report the SIM card lost so that it is deactivated.

Yeah, just go into the store and tell them that you lost your SIM card. They will give you a new one and program your current number into it. You may lose all of your contact numbers though if they were only stored to you SIM card.
